Far North Credential Check

Wednesday July 11th from 5:00-7:00pm at AT&T Sports Center

Competitive Division Teams - Managers bring; signed player registration forms** for all players, copies of birth certificates for all players, all player passes, and a tournament roster* (5-6 copies).

U-10 Recreational Division Teams - Managers bring; signed player registration forms**, copies of birth certificates, and tournament roster* (5 copies).

U-8 Coed Recreational Division Teams - Coaches or Managers bring a binder with signed player registration** forms for all players, copies of birth certificates for all players, and a printed tournament roster* with team name, player names, date of births, jersey numbers (5 copies).

*Rosters - Official tournament rosters created by the manager/coach and are brought to credential check. This will be the roster to be used for the Far North Invitational. T-Shirt rosters must be emailed to the Tournament Director no later than Monday June 25th.

**Click below for an example of a player registration form. This needs to be signed and dated after September 1, 2011 on all players in the tournament.
